ডাক্তার যুক্ত করার অনুরোধ করুন
DaktarInfo-তে কোনো ডাক্তার এখনো তালিকাভুক্ত না থাকলে এই পেজ থেকে তার Doctor Profile যুক্ত করার অনুরোধ পাঠাতে পারেন।
ডাক্তার নিজে, হাসপাতাল বা চেম্বারের প্রতিনিধি, অথবা একজন রোগী/সাধারণ ব্যবহারকারী সঠিক তথ্য দিয়ে Doctor Profile Request submit করতে পারবেন।
গুরুত্বপূর্ণ: Form submit করার সঙ্গে সঙ্গে Doctor Profile প্রকাশিত হয় না। প্রথমে তথ্য DaktarInfo-এর administrator-এর কাছে review-এর জন্য যাবে। প্রয়োজনীয় যাচাই ও approval-এর পর profile public Doctor Directory-তে প্রকাশ করা হবে।
কীভাবে কাজ করে?
Doctor Profile যুক্ত করার প্রক্রিয়াটি তিনটি ধাপে সম্পন্ন হয়:
১. Doctor Information Submit করুন
ডাক্তারের প্রয়োজনীয় পরিচয়, BM&DC number, qualification, specialty, workplace, hospital এবং অন্যান্য তথ্য form-এ পূরণ করুন।
২. DaktarInfo তথ্য Review করবে
Submit করা profile প্রথমে Pending অবস্থায় থাকবে। Administrator তথ্য review করবে এবং প্রয়োজন অনুযায়ী সংশোধন বা verification করতে পারবে।
৩. Approval-এর পর Profile প্রকাশিত হবে
তথ্য গ্রহণযোগ্য হলে administrator profile Publish করবে। এরপর Doctor Profile DaktarInfo-এর public directory এবং সংশ্লিষ্ট specialty/location page-এ দেখা যাবে।
কারা Doctor Add Request করতে পারবেন?
নিচের যেকোনো ব্যক্তি request submit করতে পারবেন:
Doctor নিজে
Doctor নিজের profile যুক্ত করার request করতে পারবেন।
Form-এর:
Request submitted by → Doctor (Self)
নির্বাচন করুন।
Hospital বা Chamber Representative
কোনো hospital, chamber বা clinic-এর authorized representative doctor-এর information submit করতে পারবেন।
নির্বাচন করুন:
Hospital / Chamber Representative
Patient বা সাধারণ ব্যবহারকারী
কোনো doctor DaktarInfo-তে না থাকলে একজন patient বা সাধারণ ব্যবহারকারীও accurate information দিয়ে request করতে পারবেন।
নির্বাচন করুন:
Patient / Member of the Public
তবে public submission সরাসরি প্রকাশ করা হবে না। Administrator review করার পরই profile publish করা হবে।
Doctor আগে থেকেই DaktarInfo-তে আছেন কি না দেখুন
নতুন request করার আগে Doctor Directory-তে doctor-এর নাম search করার পরামর্শ দেওয়া হচ্ছে।
Doctor-এর existing profile থাকলে নতুন profile submit করবেন না।
বিশেষ করে একই BM&DC Registration Number দিয়ে DaktarInfo-তে existing Doctor Profile অথবা Pending Request থাকলে system নতুন request গ্রহণ করবে না।
এতে duplicate doctor profile তৈরি হওয়া কমে।
১. Doctor Identity
প্রথমে doctor-এর basic professional identity দিতে হবে।
Required information:
Doctor Full Name in English *
ডাক্তারের পূর্ণ নাম English-এ লিখুন।
উদাহরণ:
Dr. Mohammad Karim
এই field-এ বাংলা নাম ব্যবহার করা যাবে না।
Doctor’s Name in Bangla *
ডাক্তারের নাম বাংলায় লিখুন।
উদাহরণ:
ডা. মোহাম্মদ করিম
Gender *
নির্বাচন করুন:
- Male
- Female
Designation & Department *
ডাক্তারের বর্তমান পদবি ও বিভাগ বাংলায় লিখুন।
উদাহরণ:
সহযোগী অধ্যাপক, কার্ডিওলজি বিভাগ
Affiliation / Workplace *
ডাক্তারের বর্তমান প্রধান কর্মস্থল বাংলায় লিখুন।
উদাহরণ:
জাতীয় হৃদরোগ ইনস্টিটিউট ও হাসপাতাল
BM&DC Registration Number *
ডাক্তারের BM&DC Registration Number অবশ্যই দিতে হবে।
উদাহরণ:
A-14000
BM&DC Number ছাড়া নতুন Doctor Profile Request submit করা যাবে না।
BM&DC Number কেন Required?
BM&DC Registration Number doctor-এর professional identity শনাক্ত করার জন্য DaktarInfo-এর গুরুত্বপূর্ণ reference।
একই BM&DC number আগে থেকেই কোনো:
- Published Doctor Profile
- Pending Request
- Draft Doctor Profile
- Private Doctor Profile
এর সঙ্গে যুক্ত থাকলে নতুন submission block করা হবে।
আপনি যদি existing profile-এর doctor হন এবং profile update করতে চান, নতুন Doctor Request না দিয়ে Contact Us বা Profile Claim/Correction process ব্যবহার করুন।
২. Education & Specialties
Doctor-এর professional qualification এবং specialty information দিন।
Education / Degrees *
ডাক্তারের degree ও qualification বাংলায় লিখুন।
যেমন:
এমবিবিএস, বিসিএস (স্বাস্থ্য), এমডি (কার্ডিওলজি)
এটি Required field।
Alumni Of
Doctor কোন medical college, university বা institution-এর alumni হলে উল্লেখ করতে পারেন।
Fellowships / Trainings
Professional fellowship এবং গুরুত্বপূর্ণ training-এর তথ্য দিতে পারেন।
Specialist Area
Doctor কোন কোন বিষয়ে বিশেষজ্ঞ তা বাংলায় লিখতে পারেন।
যেমন:
হৃদরোগ, উচ্চ রক্তচাপ, হার্ট ফেইলিউর ও ইন্টারভেনশনাল কার্ডিওলজি
Department নির্বাচন করুন *
অন্তত একটি Department নির্বাচন করা বাধ্যতামূলক।
প্রথমে Main Category নির্বাচন করুন।
প্রয়োজন হলে তার Subcategory নির্বাচন করুন।
তারপর:
+ Add Department
click করুন।
একজন doctor-এর একাধিক relevant department থাকলে একাধিক department যুক্ত করা যাবে।
৩. Hospital & Chamber Information *
প্রতিটি Doctor Request-এর সঙ্গে অন্তত একটি Hospital বা Chamber যুক্ত করা বাধ্যতামূলক।
Hospital নির্বাচন করার জন্য:
Division → District → Thana → Hospital
অনুসরণ করুন।
Division
প্রথমে hospital-এর Division নির্বাচন করুন।
District
তারপর District নির্বাচন করুন।
Thana
প্রয়োজনে Thana নির্বাচন করতে পারেন।
Hospital
তারপর DaktarInfo database থেকে hospital search করে নির্বাচন করুন।
একটি hospital select করার পর Add button ব্যবহার করে doctor-এর profile-এর সঙ্গে যুক্ত করুন।
একাধিক Hospital যুক্ত করা যাবে?
হ্যাঁ।
একজন doctor একাধিক hospital বা chamber-এ রোগী দেখলে একাধিক hospital যুক্ত করা যাবে।
বর্তমান system সর্বোচ্চ ১০টি hospital/chamber entry process করতে পারে।
প্রতিটি hospital-এর জন্য প্রয়োজন অনুযায়ী নিচের information দিতে পারবেন:
Appointment Phone
সেই hospital/chamber-এর appointment number।
Hospital Address
প্রয়োজনে doctor-এর chamber-এর specific address লিখতে পারবেন।
Schedule
Doctor-এর patient visiting schedule লিখুন।
উদাহরণ:
Saturday to Thursday, 3:00 PM to 9:00 PM
Closed: Friday
Doctor-এর Location কীভাবে নির্ধারিত হয়?
Doctor Request approve হওয়ার জন্য selected hospital গুরুত্বপূর্ণ।
Doctor-এর সঙ্গে যে hospitalগুলো যুক্ত করা হয়, DaktarInfo system সেই hospital-এর location information থেকে doctor-এর relevant:
- Division
- District
- Thana / Area
সম্পর্কিত location data sync করতে পারে।
তাই সঠিক hospital নির্বাচন করা গুরুত্বপূর্ণ।
৪. Contact & Practice Information
এই section-এ doctor-এর professional ও private contact information দিতে পারবেন।
Doctor’s Personal Mobile Number *
Doctor-এর personal mobile number Required।
এই number public profile-এ সরাসরি প্রকাশ করার জন্য নয়। এটি administrator-এর জন্য সংরক্ষিত থাকে।
Telephone
Optional।
Admin-only information হিসেবে রাখা হতে পারে।
Optional।
Admin-only information হিসেবে রাখা হতে পারে।

Private Information Public হবে কি?
Doctor Request form-এর:
- Personal Mobile

- Personal Email
administrator-only information হিসেবে সংরক্ষণ করা হয়।
এগুলো form submit করার কারণে automatically public Doctor Profile-এ প্রকাশ করা হয় না।
এগুলো verification এবং administrative communication-এর কাজে ব্যবহার করা হতে পারে।
Website
Doctor-এর official বা professional website থাকলে URL দিতে পারেন।
উদাহরণ:

Consultation Fees
Available থাকলে consultation fee information দিতে পারেন।
Fee সময়ের সঙ্গে পরিবর্তিত হতে পারে, তাই যথাসম্ভব current information দিন।
Started Practicing Year *
Doctor কোন বছর থেকে medical practice শুরু করেছেন সেটি দিতে হবে।
উদাহরণ:

বর্তমান system অনুযায়ী year অবশ্যই 1950 থেকে বর্তমান বছরের মধ্যে হতে হবে।
এই তথ্য থেকে doctor-এর approximate professional experience system calculate করতে পারে।
Languages
Doctor কোন কোন ভাষায় patient-এর সঙ্গে communicate করতে পারেন তা দিতে পারেন।
যেমন:
Bangla, English
Doctor-এর professional Facebook profile/page থাকলে URL দিতে পারেন।
Professional LinkedIn profile থাকলে URL দিতে পারেন।
৫. Doctor Profile Content
Doctor সম্পর্কে আরও helpful information দিতে পারবেন।
Doctor Biography / About
Doctor-এর professional biography বাংলায় লিখুন।
এখানে থাকতে পারে:
- Professional background
- Qualification
- Current position
- Areas of expertise
- Experience
Diseases / Conditions
Doctor সাধারণত কোন কোন রোগ বা condition নিয়ে কাজ করেন তা বাংলায় উল্লেখ করতে পারেন।
এখানে শুধু accurate professional information দেওয়া উচিত।
Doctor Photo *
একটি Doctor Photo upload করা বাধ্যতামূলক।
Supported image:
- JPG
- PNG
- WebP
Maximum file size:
2 MB
Uploaded image DaktarInfo system দ্বারা WebP format-এ convert করে সংরক্ষণ করা হয়।
সঠিক doctor-এর পরিষ্কার ও professional photograph ব্যবহার করুন।
অন্য ব্যক্তির ছবি ব্যবহার করবেন না।
৬. Submitter Information
Doctor-এর information কে submit করছেন সেটিও আমাদের জানতে হবে।

Your Phone *
একটি valid phone number দেওয়া বাধ্যতামূলক।
প্রয়োজনে submitted information verification-এর জন্য DaktarInfo আপনার সঙ্গে যোগাযোগ করতে পারে।
Your Email
Email optional।
তবে verification বা communication সহজ করার জন্য একটি valid email দেওয়া ভালো।
Submitter-এর contact information public Doctor Profile-এ প্রকাশ করার উদ্দেশ্যে সংগ্রহ করা হয় না।
তথ্য প্রকাশের Consent
Form submit করার আগে আপনাকে নিশ্চিত করতে হবে যে:
আপনার জানা অনুযায়ী submitted information সঠিক এবং verification-এর পর DaktarInfo-তে information প্রকাশ করার অনুমতি দিচ্ছেন।
Consent checkbox select না করলে request submit করা যাবে না।
Submit Request
সব required information পূরণ করার পর:
Submit Request
button click করুন।
Form সফলভাবে submit হলে confirmation message দেখানো হবে।
Submit করার পরে কী হবে?
Request submit হওয়ার পর system নতুন Doctor Profile-কে:
Pending
status-এ তৈরি করবে।
এটি তখন public Doctor Directory-তে দেখা যাবে না।
DaktarInfo administrator request-এর সঙ্গে পাওয়া information review করতে পারবে।
Admin panel-এ দেখা যাবে:
- Request source
- কে submit করেছেন
- Submitter phone
- Submitter email
- Relationship
- Doctor private contact information
- Submission date
- Submitted doctor information
Administrator-কে profile publish করার আগে information review করার নির্দেশনা system-এ দেওয়া থাকে।
Admin কী Review করবে?
প্রয়োজন অনুযায়ী Administrator:
- Doctor identity
- BM&DC Registration Number
- Name
- Qualification
- Department
- Specialty
- Workplace
- Hospital
- Location
- Chamber information
- Doctor image
- Contact information
- Submitted source
review বা correction করতে পারে।
বিশেষ করে hospital এবং location information সঠিকভাবে assigned হয়েছে কি না তা review করা গুরুত্বপূর্ণ।
Profile কখন Public হবে?
Request submit করার সঙ্গে সঙ্গে নয়।

Administrator WordPress dashboard থেকে Doctor Profile Publish করার পরই সেটি DaktarInfo-এর public directory-তে দেখা যাবে।
Request Reject হতে পারে কখন?
নিচের কারণে Doctor Request publish না-ও করা হতে পারে:
- BM&DC Number পাওয়া না গেলে
- একই BM&DC Number-এর existing profile থাকলে
- Doctor identity নিয়ে সমস্যা থাকলে
- ভুল বা misleading information থাকলে
- প্রয়োজনীয় required information না থাকলে
- ভুল doctor photograph দেওয়া হলে
- Hospital information নির্ভরযোগ্য না হলে
- fraudulent submission মনে হলে
- information যথেষ্টভাবে যাচাই করা না গেলে
Submission করা মানেই publication নিশ্চিত নয়।
প্রতি ঘণ্টায় Submission Limit
Spam ও automated abuse প্রতিরোধের জন্য বর্তমান DaktarInfo system-এ একই visitor/source থেকে:
প্রতি ঘণ্টায় সর্বোচ্চ ৩টি Doctor Request
submit করা যায়।
Limit অতিক্রম করলে পরে আবার চেষ্টা করতে হবে।
Security Protection
Public Doctor Request form-এ security এবং spam protection রয়েছে।
এর মধ্যে রয়েছে:
- Security nonce validation
- Honeypot spam protection
- Submission rate limiting
- File type validation
- File size validation
- Phone validation
- Email validation
- Duplicate BM&DC checking
এর উদ্দেশ্য directory-তে automated বা fraudulent submission কমানো।
Existing Doctor-এর Information Update করতে চান?
Doctor যদি DaktarInfo-তে আগে থেকেই থাকেন, /add-doctor/ থেকে duplicate profile তৈরি করবেন না।
সেক্ষেত্রে ব্যবহার করুন:

Existing profile URL এবং সঠিক information দিয়ে update request পাঠান।
গুরুত্বপূর্ণ ঘোষণা
DaktarInfo-তে Doctor Request submit বা profile publish হওয়া কোনো doctor-এর:
- চিকিৎসার মানের guarantee নয়
- clinical skill-এর ranking নয়
- treatment result-এর guarantee নয়
- DaktarInfo-এর ব্যক্তিগত recommendation নয়
Doctor Profile মূলত healthcare provider সম্পর্কিত professional information সহজে খুঁজে পাওয়ার জন্য প্রকাশ করা হয়।
সঠিক তথ্য দিয়ে Doctor Profile Submit করুন
নতুন Doctor Profile যোগ করার আগে নিশ্চিত করুন:
✓ Doctor আগে থেকেই DaktarInfo-তে নেই
✓ সঠিক BM&DC Registration Number আছে
✓ Doctor Name ও Professional Information সঠিক
✓ অন্তত একটি Department নির্বাচন করেছেন
✓ অন্তত একটি সঠিক Hospital/Chamber নির্বাচন করেছেন
✓ Doctor-এর Personal Mobile Number দিয়েছেন
✓ Started Practicing Year দিয়েছেন
✓ সঠিক Doctor Photo upload করেছেন
✓ Submitter Name ও Phone দিয়েছেন
✓ Publication ও Verification Consent দিয়েছেন
তারপর Submit Request করুন।
DaktarInfo team review ও approval করার পর Doctor Profile public directory-তে প্রকাশ করা হবে।
